residential Kentucky customers on the purchase of high-efficiency Energy Star natural gas home heating and water heating equipment.

Program Guidelines
Please follow these guidelines to receive your rebate. Failure to adhere to these guidelines could delay or disqualify your application.
- Applicant must be an Atmos Energy customer located in Kentucky and served under either the company's Residential Firm Service G-1 Sales Service; or a home owner who is installing qualifying Energy Star equipment in a new home.
- Purchase and install eligible equipment (as stated above) in a home that is or will be served by Atmos Energy. Include a copy of an Atmos Energy bill under the customer’s name and address listed with this rebate form.
- Complete and submit the Kentucky High-Efficiency Rebate Installation and Verification Form (.pdf). You will need the following information to correctly complete the rebate form: your Atmos Energy account number, one of your recent Atmos Energy utility bills, a dated purchase receipt for the eligible equipment and a proof of installation.
- The rebate applies to new home construction or replacement of equipment in an existing home by August 31, 2010.
- Submit a separate form for each rebate requested (for example, if you purchased a qualifying water heater and furnace, fill out and submit a separate form for each.)
- Incomplete rebate applications may delay or disqualify your rebate. Please keep a copy of the completed forms and all attachments for your
records. Mail the Kentucky High-Efficiency Rebate Installation and Verification Form (.pdf) and required information to:

- For questions regarding the rebate process, call EFI at 1-877-313-8805.
- DO NOT SEND THE REBATE APPLICATION OR INSTALLATION VERIFICATION FORMS WITH YOUR ATMOS ENERGY BILL PAYMENT.
- Upon receiving both a properly completed Kentucky High-Efficiency Rebate Installation and Verification Form (.pdf), EFI will issue a check to the program participant within 8 to 10 weeks.
- By applying for this rebate, you agree to allow Atmos Energy to verify the installation of any equipment covered under this program, if necessary.

Claim the Federal Tax Credit
Atmos Energy customers/homeowners can claim the Atmos Energy rebate in addition to the federal tax credit on qualifying equipment.
Equipment efficiency standards for the federal tax credit can differ. The federal tax credit is 30% of purchase price, up to $1,500.
